Physical Examinations

Students registering for Athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts.  To ensure clearance for participation, physicals should be completed and turned in to the nurse no later than 5 school days prior to the first day of practice/tryouts. Please bring the completed physical (and authorization for medication if needed) to either WMS or WHS main office and leave for the school nurse.

Required Athletic Paperwork:

Students registering to participate in any sport offered by the Rhode Island Interscholastic League must read and sign the following four forms:

1.) Athletic Consent/Insurance Form: this form just needs to be filled out once each school year. Online document only, please do not print.
2.) RIIL 2020 Assumption of Risk Form: NEW 2020! All high school students need to fill out a new one and have notarized. Please hand into coach Day 1.
3.) Concussion Form: this form needs to be filled out once each school year. Online document only, please do not print.
4.) Current Physical: must be on file in the school nurses office before the student is allowed to participate in practice/tryouts. To ensure clearance for participation, physicals must be dated and signed by an authorized physician and turned in to the school nurse no later than 5 school days prior to the first day of practice/tryouts. Physicals are valid for 12 months from date of the last exam. Please bring to the main office of the school to drop off.
5.)  Authorization for Medication - If required to carry medication at all times, please have this form filled out by your physician as well. Please bring to the main office of the school to drop off.
